  Please note the following instructions for printing out forms:

1. All forms print on 8.5" x 11" standard paper and cut to size.

2. Make sure all required signatures are on the forms prior to sending them to your  district commissioner for

  approving and signing.

3. Make every attempt to use actual forms and only use these printable forms in emergency situations.

Any Alabama ASA team that will be participating in a Regional or National Championship level tournament MUST use the official ASA forms below.  All forms MUST be signed by the State Commissioner before being sent to the tournament site.

These 2 forms are for teams to use prior to playing in Champioship level tournaments.   Once a team begins Championship level participation these 2 forms can no longer be used.   The ASA Roster must be completed and signed prior to any Championship play.  Once a team plays in Championship competition the roster is closed and the only way to add additional players to a roster is with the ASA Pick-up Player Form (see Article 503 A - C of the ASA Code), and then only three players may be picked up.

When Printing the ASA Roster it needs to be printed on one sheet of paper front and back.  Two sheets will not be accepted as a completed roster.

The Pick Up Player Form can only be used after the Final Qualifying Tournament has been played (usually the State Tournament).  Players can only come from teams that are no longer eligible for Championship Play.

Pick up Players Must have been on an ASA roster in the current year and must come from same division of play or lower.
